This link takes you to a form on a webpage that will help you select classes for your students. You may print the form, complete it and mail it back to us with your signed contract and deposit or use it as a worksheet and then email or call us with your class selections.
The ES237 Attendance Registration Form data
Complete the ES-237 Attendance Registration Form before arrival and present it along with your payment at the office when you register. This form must be completed in order to pay attendance fees and register your group. A paper version of this form is located in the back of the hard copy version of the Trip Planning Guide for Teachers.
